Dos this mouse come with the usb receiver?

yes it does, but no batteries. the one i bought was also refurbished, but it was a faulty one. Heard this particular model had a lot of complaints from users

Does the button behind the scroll wheel switch the scrolling from infinite to 'lines at a time'?

yes.small size for small hands. the push button allows to scroll freely or select lines at a time or page a a time. need to program it for functions 
i like the performance mx unit better for me, larger size better fit more functions.logitech are the better units made.

do you use ANY type of AA battery, or hi-efficiency ones?

I use your regular duracell or energizer.  Batteries last forever in this mouse.  Especially if you use the switch on the bottom to turn it off, which I don't.  I can't remember the last time I changed the battery.  Probably a year ago?  I bought a second one of these just as a backup to the one I bought a couple of years ago.

Does this mouse have the side thumb button?

Yes, it has the lower thumb button. It also has two higher thumb buttons. The regular left, middle, and right buttons. The wheel toggles left and right. That makes the 8 buttons.

Does it come with batteries?

Sorry, but you will have to supply a pair of AA batteries yourself.  Otherwise, this is a great mouse, works perfectly with my wireless Logitech keyboard.  With the Unifying feature Logitech offers, you only need one antenna (one USB port) for both devices.
Try the smooth scrolling if you like ... I found it irritating actually and later configured the mouse to scroll 1 line at a time.  For the price, this mouse is a winner!
