Does this come with an HDMI to DVI adaptor?

it does not have any kind of adaptor or cable, but it does have hdmi port, dvi port and display port and you can use all the 3 together!

Is this GPU fit for a slim PC?

You have to click the option button "Double Fans" then the option button ending "...2 GT LP" to get the shape you want.
