If I got this board, what are the chances it would be DOA?  Also, how many fan headers does it have? (not including the CPU fan header.)

There are 2 Chassis Fan headers. As for chances of it being DOA, I didn't have any issues with mine, works just fine.

The hookups for the Power Switch are on the right hand side as you look at the board from the back, just to one side of the PCI-E slot, along with hdd light, speaker connector and reset.

With an interface module, yes, but you loose a couple of Sata Ports in the process
