Does this come with the screw to screw it down on the stand-off?

This 100% did not come with a screw. I just got mine a week ago. I had to use a stand-off and screw set that came with my motherboard.

anyone have advice on a heatsink for this? not finding options for a dual sided m.2

Heat sinks are not always necessary., it really depends on the environment which your PC lives and the use case of the machine. If you are doing extremely data intensive tasks, such as video editing, these drives will get hot. That said, NVME's do not like the cold, infact if they run too cool they will not run well.

Mushkins Enhanced Pilot m.2 SSDs are a little thicker( just a few microns) than other brands so it can be difficult to get a snap on cooler to fit the drive. It is possible to use the more common snap on style coolers (like Advancing Gene)  if you trim down the thermal pad thickness, however it may be best to use a cooler that is held together with a band or clip if you really need to use a cooler on this drive.

will this work in asus prime z490-a? it's not on the manufacturer's approved list.

It really shouldn't be a problem. The PC recognize it the same any other M.2 Drive you plug in. M.2 SSD's are not like RAM where you have to have very specific model numbers to ensure compatibility, the drives use a standard protocol over the PCI bus so as along as the slot works and the drive is good, you shouldn't have anything to worry about.
