I see it has some type of insert. Is this for 2.5 drives? and does the insert come out to load a 3.5 drive?

It's not an insert. The small open slot on the bottom right is for 2 1/2" drives. They go most of the way in. What you call an insert is actually a door, hinged at the top. It swings up inside the case when you insert a 3 1/2" drive. A 3 1/2" drive hangs out of the case a couple of inches.

Does this support 1TB drives?

Actually, I at first didn't notice the description stating it supports up to 6TB drives.

Question 3

This has both Molex and SATA power connectors. Is this an either/or option, or do both have to be connected?

It's option, you only connect one connector.
