Do you have to have exactly 750 watt PSU??

No 750w or Greater is the recommendation, don't do it with anything under 750 though. Personally I have had nothing but problems since I purchased this card, crash dumps, black screen startup issues. I have other cards that I put in and have had nothing but issues and am on my second RMA of this card. Look elsewhere.

does the video card look exactly like the one on the picture ?

No, unless they have changed something recently the card looks like the majority of the R9 290s just rebranded. The corners on the photo are much shaper where the actual card has curved corners. Maybe this is a newer photo? Not sure but it is not a photo of the card I received.

It says this card is a 4GB 384-bit GDDR5 but aren't all r9 290's 512-bit?

Yes they are, so either this is a typo, or Diamond went cheap and made it only gave it a 384-bit wide memory interface [so it isn't truly an R9 290].
