Yes, an adapter is included in the box.  Nice card.  Great heat sink.  And silent.  Good finish on the parts.  Even a plastic clear removable protector over the ZOTAC on the heat sink.

Is this card GDDR3 or DDR3? I have asked Zotac and they stated it is GDDR3 as well as Newegg states although everywhere else I look it's only listed as DDR3. Not only is it listed on other websites as DDR3, GPU-Z states it as DDR3. If it is DDR3, that's technically false advertisement and I will be returning this product if so.

I bought this exact card from Newegg and received it 1/3/2014. The memory on it is Micron with FBGA Code D9PRV which translates to Part Number MT41J128M16JT-125G:K. It is GDDR3-1600.
While the memory supplier may vary throughout the production life of any given card, the specifications will, with rare exception, remain consistent and in this case regardless who makes it, the memory will always be GDDR3-1600 and limited to that rather than GDDR3-1800 or GDDR3-2000 due to passive cooling.
As a side note - as you browse various product overviews and spec sheets you'll see inconsistent use of DDR3/GDDR3; many manufacturers, resellers, reviewers, and even programs have adopted the understanding that when it comes to a graphics card, DDR3 is synonymous with GDDR3.

Question 4

Anyone know the size of the card ? In the specs it doesn't say and i was hoping to install this in a silent mini itx build but i dont know if its less than 170mm?

I have this card, it is bigger than it looks.  The brochure for it from Zotac states the card is 111.15mm high by 146.86mm wide.  The cooling fins wrap around the back of the card and can push up against the ram tabs on some boards, preventing you from installing it.  I have mine in a mid-ATX case and it forced me to shuffle some hard drives around to fit.

If you still want a silent Zotac, consider a step down to their GT 630 model depending on your application use.  It is ZT-60409-20L.

Question 5

Just want to verify because pictures are inconclusive - do the cooling fins extend beyond the back of the card? If not, then it should just clear the memory slots of my mobo.

I'll "try" and answer. Measure the length you need to the length advertised. Don't rely on pics. The trouble here is on my motherboard the memory slots are next to the CPU and the PCI slots have no obstruction.  I did note if the memory slots were positioned behind the PCI slots, it could hit the first men slot... I've seen far less video cards with LOUD fans twice this cards size. Keep in mind this card will seat in one PCI slot but it will render the slot below it useless. It is a big card. I personally love the card. Zotec is very high quality, and I'm very happy with its hand made looks, and just how seriously powerful it is!!

Work with Windows 8?

I installed into vista, but YES... this is a modern vidcard by Nvidia and has working drivers for all Win OS's.  The price is low because it has little to no value for gamers , due to passive cooling & a requires slightly more power than was shipped on the average Dell Desktop, requiring a PSU upgrade.  Making it a GREAT value for an HTPC machine or photoshop machine.  It will drive 3 monitors! 
BE ADVISED:
1) YOUR machine must have a modern case with an airslot ABOVE the top pci-e slot (I modified the card by removing the attached slotcover)
2) Air fins can be bent slightly to fit around cpu coolers & obstructions.
3) Buy a 15+ft mini-hdmi to hdmi cable (here on newegg) the adapters sold in your local store will not fit.  The plastic case is too large and will hit obstructions on the back of your case or DVI connection.  The new cable is inexpensive and has a small plug to fit into the restricted opening.

Does this card support Blue Ray?

This video card is higher resolution than the 1080p resolution of blu-ray.  This should work great.

Does this video card support duel monitors? If not what is the most inexpensive but still good performance VC that will I am running a intel setup

GT 640 and above support 3 monitors on one card.  Lack of a cooling fan suggests limited FPS gaming function regardless of the capabilities of the GPU itself, due to inevitable overheating.  It works very well driving 1080p content and is absolutely silent.

Will a 300w power supply on a desktop be enough for this card?

According the Zotac website, this card draws 65-watts max, and is recommended for use with a PSU with at least 350 watts, but a good quality PSU with 300 watts will probably work OK.

Per the Zotac Website, a power supply of 350 Watts minimum is recommended. This card does not require a PCI-E power connection (obtains all power from the motherboard connection).
