Does this bracket adapter comes with any screws/nuts?

Mine did.  Look for two little separate zip lock baggies with 4 screws each.

Does this come with a cover for the 3.5 bay hole?

no.  exactly as pictured, but fits well and very adaptable to different drive applications.

Question 3

How is the texture of the front of this thing? is it glossy? or matte?

the case i use has a matte finish. it's really like a black metal, but plastic usually looks just fine if it's a textured finish.

what i don't want is smooth/glossy, nor do i want brushed metal look.

It's definitely smooth and glossy. If you want it flat black, you can just use a little spray paint.

I installed mine in a Phantex Enthoo Pro, which has a black brushed-metal appearance. I scratched a bunch of vertical lines with an Xacto knife, then used some generic black acrylic paint--using vertical strokes. It's a very good match.

Question 4

Will a zip 250 MB internal drive fit in this enclosure?

If a Zip 250MB internal drive fits into a standard floppy drive bay, then yes, it will fit in this. The card reader that I have installed in mine is 3 15/16" (10.1cm) wide and 15/16" (2.4cm) in height. I believe almost all card readers are the same size as a floppy drive.
