No, I've purchased several M.2 drives from Samsung and WD, they never come with that tiny screw.

The heatsink on this model is thick and didn't fit on my ASUS motherboard. I'd recommend getting the non-heatsink version.

Is the heatsink just a gimmick or does it actually work in this case? Has anyone compared this directly with the non-heatsink version?

The heatsink is certainly not a gimmick. The majority of drives that fail do so because of heat, these drives get hot. Most motherboards come with heatsinks for the m.2 slots because they realize these drives need to kept on the cooler side.

I just ordered one of these with a heatsink, because I didn't realize that my motherboard had built-in heatsinks for these ports. (I'm new to PC building.) Should I leave the pre-installed heatsink on, or is it better to remove it and use the motherboard heatsink?

Return it and get one without the heat sink. If you remove that heatsink you will void the warranty.
