Has anyone tried using it in Windows 7 Pro 64bit?

On a Lenovo M57 with a static IP address, it did not work as a SECOND port. I did not try it as the only network port (onboard port disabled). My application required 2 ports, both with fixed addresses. The system recognized the PCI card was there, but I could not get it to communicate outside the box with the static addrees. It did work with a dynamic address.

Will this adapter work in my HP Pavilion Slimline s5-1204 PC?

No you will need a pcie (pci express) network card. It will half to be a Half height card / low profile as well.
