Does this phone come with sim card?

I got the SIM card with the phone, but when I switched to T-mobile service I got a new SIM card. Perhaps because it was more than a month since I bought the phone and switched to T-mobile.

Can this phone be used at home as just a wifi device without activating it with T-Mobile or unlocking it? I would like to buy it to use as an MP3 player but I don't want to have to activate it or anything like that before I use it.

Yes, you can use the phone as a wifi device or an mp3 player without putting a sim card in it (aka "activating it" with T-mobile).

Be warned however, I had a really bad experience with this phone. The Lumia 521 is known amongst customer reviews for its poorly made hardware. My phone personally was prone to shut down whenever it wanted, and I had to buy another phone (not the Lumia 521) after just a few months of using it.

Does this phone have "Hot Spot" capability?  Can this phone create a "HOT SPOT", allowing other device to connect to the internet using this phone?  My samsung s5, and iphone has this capabilities.

I have never tried it, but the User Guide says you CAN do this.  Here's a quote from the User Guide:

"Want to use the internet on your laptop but don't have access? You can use your phone's mobile data connection with your laptop or other devices. A Wi-Fi connection is then created . . . The other device uses data from your data plan, which may result in data traffic costs."

I'm not sure if it is carrier unlocked, I know IT IS NOT the pay as you go version of the 521.

I've only used it on T-Mobile.

You could call T-Mobile and ask if their 521 is locked by default and based on their answer assume that this one is set to the default.

Is this phone unlocked?

Only T-Mobile Micro SIM cards will operate this phone is my understanding.  It comes with a T-Mobile prepaid Micro SIM card that needs to be activated.  One can also use an existing T-Mobile SIM card which is what I used.  I used a SIM Card cutter to cut the SIM card down into a Micro sized card.  One must be careful doing this so as to not damage the card.
I have not tried to use an AT&T SIM card.  It has software Apps which are designed for  the T-Mobile network.

No, it is not unlocked, but it is flexible.  You can only use it on T-Mobile, but with them you can sign up for a long contract or prepay on a month-to-month basis (pay-as-you-go).
