Yes, it does.  And, I have had great success with Pyle products over the years. This is no exception.  It performs as it should and as advertised.  I tested it out as soon as I got it, and all 4 channels work perfectly. You can't go wrong for the price !

Question 2

If I am coming out of a headphone output (of an audio interface in this example) and then going into the input of this device, what kind of cord should I use?

The audio input of this device uses a standard 1/4" headphone jack, so any appropriate cord similar to that used by headphones will do. Personally, I'm using a 3.5mm cord and plug that's connected to the input of this device using an easy to find adapter to fit into the 1/4" hole.
