Will this work with the h.264 4ch DVR?

I don't know which exact DVR you have but if it accepts 'standard' NTSC video, 75 ohm impedance, around 1v signal it should produce video. 
H.264 is a compression/format standard that the a DVR supports similar to MPEG4 and MJPEG which has little to do with basic camera compatibility.

Short answer, most H.264 DVR can use NTSC video signals around the 1v/p-p range but you should verify the DVR spec against this camera to make sure.

On the plus side, I have owned this camera for about 3 years, connected to an 'HD' DVR (displays in HD, records in much less) but the camera has been reliable in an area prone to lightning produced power surges and other problems. 
Cam survived a lightning hit that took out other equipment on the same ckt. Cam only needed a power cycle to recover and it is not on UPS grade surge protection. That is Worth the money for me. 
Resolution is not too high and contrast/sharpness is not the best but the price reflects this.

Does camera include connection cable?

No. It needs to be hard wired to a DVD surveillance recorder with coax cable. My DVD is located in the house and the camera located above the garage door. I needed 60 feet of cable. You will also need a DC power supply for a 120v AC house current.

Camera works good, even at night.

Question 3

This comes without a power adapter, what is the min and max amps?

What's the power requirements for this thing? Would I be able to run five off of a simple plug in/splitter such as this - http://www.newegg.com/Product/Product.aspx?Item=9SIA1R30NS0955 Or will I need something a bit more heavy duty like a transformer box?

I have the Kguard ot-801 which is no loger availible and I have this to replace two of the camera that went out on me. If you look that Up it will give you all the spec on it. I am not sure what yiou are trying to do with a splitter?
