Question 1

Are the PCI slots permanent punch outs or can they be put back into place?

They are removable/replaceable and are held in place by screws and an actual cover that goes over those that you can use a lock on. It's a great case and is designed very well. I was shocked at the quality of the case overall. I highly recommend it. The fans are quite good as well, I have them at one back and two at top (exhaust) and added two ninja scythes to the front (intake). Everything stays nice and cool even with case fans on the lower of the two settings, little loud but that's due to the Scythes on high it sounds like a jet. It's not a quite case but airflow is perfect as I have it set up. It is Micro or Mini ATX Only. Good luck with your build.

Question 2

Does this require a specially sized PSU? How do you install it?

You can actually use Full Sized PSUs.
Anything over 750watt might be a squeeze though. So as long as it is a standard size and length you should have no issues. I'm running a standard Corsair 650watt in mine. You may want to go ahead and install the cables you'll need connected to a modular unit before installing the PSU, makes things much easier.

Question 3

I bought this case and for some reason my build will not work in it, I start it up but it won't boot. Spacers were put on the motherboard screws and everything, am I missing something? My build still works fine in my old case, please help.

Make sure you have the front panel connectors exactly where they need to go on the front panel header. The "case" won't prevent your computer from starting, if all of the power connectors are hooked up correctly.
If you suspect ANY part(s) of the case at fault, disconnect them one by one until you find the "bad part". Disconnect the USB cables and connectors... one might be bad, or possibly shorted out if something was inadvertently plugged in incorrectly. Try unhooking the fans too. 
If you take all the parts out of the case and hook them all up and it boots, THEN you might have a bad component in the case.  Good luck!

Question 4

how do I take the front panel off?

There's an opening at the bottom of the front cover. Get a screwdriver long enough to insert it and pry the bottom of the panel and the frame of the case on the left or right. Use something to cover the tip of the screwdriver or you might scratch the frame. It's covered by the front panel, but you probably don't want to scratch it, regardless.
The panel will come off a little easier each time you pry it off, and I've taken mine off a few times, but I LIKE it that it's hard to come off.
Careful when taking the cover off... the wires to the front panel headers are attached.

Is the PSU shroud removable?

Not sure as its been awhile since that rig was built. It may be removable, here is part of my review, 
Cons: Gotta have cons right. The power supply cover was missing 2 screws at the HD cage end. None of the screws I had on hand would fit tight so 2 extra motherboard stand offs were used to hold them snug.

how is the cable manegement in this case and also how long are the fan cables?

Cable management is very good. The fan cables are long enough to reach the front of the case where I have them connected to an RGB controller. This case simply blows me away on how much room there is. I am running an MSI Sea Hawk EK water cooled GTX 1070 along with a 240mm (front) radiator and a Neo pump/reservoir. Have a Deep Cool massive CPU heat sink in as well! For a micro build I rate this case a 10. BTW I am a 25 plus year veteran tech so been building for many years now. Best of luck!

Will this case support the Corsair Hydro Series H105 Extreme?  I know it says two 120 mm fans (Top), but is there enough mounting room so that it wouldn't overlap/interfere with cpu or memory?

Checked my unit for size.
With the top fans installed (2 120x25mm) there is about 40 mm of space to the top of the memory clips. I checked the size of the Hydro and that radiator is 38mm thick, a little tight, but, if installing a top side radiator system you can gain 10mm of space by using 15mm fans. Also, if you decide on a radiator system, the system must be installed to the rear of the case since the upper DVD player is only about 25mm from the top fans. This is a nice case, wish it was a little bit taller though, then a mid size board could have be used as well as a micro.

Does this come with a fan splitter for all of the fans or do I have to buy one?

His has 3-4 fan connectors that are controlled by the switch on the front, and is powered by a Molex connector. If you want to power them from the motherboard, you might need fan splitters, but different motherboards have different numbers of fan connectors

What are the fans make/model? I would like to buy 2 more for it.

The fans in my case have no other distinguishing marks than a SAMA label.  No make or model printed on them at all...

Is the Card reader USB 3.0??

note. just because it plugs into a usb 3.0 header doesn't mean its really Usb 3.0...

Sama is not a major brand name and I am unable to find detailed specs anywhere on the web.

I personally have never used the SD card reader, so I cannot attest to its speed or reliability, but I do know that it communicates with the motherboard through a USB 2.0 connection, so at best it will run at USB 2.0 speeds.
