My box came with the unit, remote with batteries, user manual, Aux Audio Cable (non-HDMI). I had bought a refurbished unit however.  The retail unit is supposed to come with the remote/batteries, bracket for wall mounting, user manual, aux audio cable, and a USB cable for connecting other devices short range.

The unit has worked wonderfully so far, but in my scenario took a few tries to get my functionality right only due to the PC powering my entertainment center wanted to see the the unit as a monitor as well causing me to lose touch screen capability in my main monitor until I sorted out the setup. For a TV this is simple plug and play.

should i invest in the replacement plan?

Not recommended.  The cost of a new one and the extreme likelihood that nothing will go wrong for many years makes the plan a poor investment.  Kinda' like collision insurance on a 2000 model year car

Power spec says Single voltage (220V). Does that mean I need to plug it into a 220V source?

I can't say I have seen the 220v spec you refer to in my paperwork.

I plug mine into a standard surge suppressor strip. The suppressor strip is first plugged into the standard 120V wall outlet
I am not sure where you saw a power spec for a 220v source. I definitely would not plug mine into a 220v source.
Normally 220v has a different plug configuration and would not accept the standard 3 prong designed for 120v wall outlets.
