Are the pictures correct on this? I don't see the 12VHPWR/PCIe 5.0 connectors but Seasonic's website links this PSU. The pictures of the ports on the PSU are clearly different. So is this not the ATX 3.0 one?

The PSU includes 2 dual 8-pin to 12VHPWR adapter cables. It is not an ATX 3.0 PSU.

Does this version come with braided cables and cable combs? Seasonic shows braided cables and combs on their website for the 1600w version.

The cables are not braided, but the plastic wire sleeves are textured to look like braided cables. A pack of clear cable combs are included.

I noticed this PSU supports up to 220V (usually EU standard) but ranges down to US standard of 120V. 1) I wanted to verify, will this product will work in the US? 2) If so, this product will maintain its efficiency rating with only 120V? Thank you.

Yes, it's still titanium rated at 120v. google seasonic 1600 tom's hardware for the full review with graphs of 115v vs 220v.
