Has anybody used this board with an expansion card installed in the top PCIe x16 slot? Does your CPU cooler prevent you from installing a card in that slot? Which cooler did you install?

I have this board currently in use with all three PCIe slots filled, though I am not using the 16x for a graphics card which is likely why you are asking.  You will likely run into the cpu cooler with a large graphics card.

I want to use the AMD - OPTERON TWELVE-CORE THIRD-GENERATION 6174 2.2GHZ 6MB L2 CACHE 12MB L3 CACHE 3.2GHZ HTS SOCKET G34(LGA-1944) 45NM 80W PROCESSOR with this board in my home network. Will 'Windows 7 Home Pro' work or will I need some version of Windows Server ( i.e. 2003, 2008 or 2012) for my OS ?

All 32 bit Windows 7 versions support up to 32 "cores" (though the number of physical chips varies by versions).. 64 bit is 256 cores..  since this is a single socket board.. you will be fine with any variant of Windows 7.  I'm not sure why you would consider running a "desktop class" OS on this board given it lacks certain desktop features (onboard sound,etc).  If you are setting up some sort of server, this is an excellent board and if you don't really need 12 cores, you can find older generation Opteron 6128 CPUs that work great in this board on e-B-a-y for cheap (less than $50)

Does this work on Ubuntu Server 14?

I see no reason why it shouldn't.  I have run Debian 7 and 8 (which Ubuntu is based on) in both server and workstation roles on this board and it works perfectly.

Question 4

What heatsink are you using with this board?  I can't tell what type it will accept and the "recommendations" are all over the place

It comes with brackets installed so you can use the clip-on type coolers.  You can remove them if you need to use the low profile screw-down opteron heat-sinks.

can i use the AMD Opteron 6380 Abu Dhabi 2.5GHz 16 MB L2 Cache 16MB L3 Cache Socket G34 115W Server Processor with this motherboard?

should be fine you can check supermicro website to be sure i built a 16 core 2.4 gig system and it screems the only drawback i found was finding video card to work with that board its older board and new stuf wont work in it  and and watch the pci slots the card slot is turned around not your normal pci slot (white slots)
