Does the board come with fan for heat sink?

The board only comes with the heat sink and you'll need to ensure that adequate air flows across the heat sink.  Otherwise the board will not operate reliably.

One review states this is for use with Supermicro cases only - which case is recommended for this?

In general you can use whatever case you want but it is important to ensure that decent airflow goes over the processor heatsink.  Supermicro's cases for this board should do a good job of ensuring airflow and that is probably the reason for the unnamed author saying that.  Do not rely on the heatsink being sufficient without air blowing across it because the processor will not work reliably without airflow.
