Is it battery or plug in? If battery how long does it last?

It's a plug in, no battery. I have to mention, unless there has been an update to firmware, the product is semi-useless for me. Some use it as a "nanny cam" or "puppy cam" and for that it works. However, I can't understand why they don't include a function to turn on and record when something trips (comes into view or sound) of the system? This would give it security cam capability. It performs as advertised for that it gets "stars". However, it's a product with virtually no home/use. I did purchase my camera quite a bit less expensively, nearly half.

do you have to pay a monthly fee to use it?

No.  But you only get limited cloud functionality.  I do not pay a monthly fee and mine is setup to send me an email with 4 still pictures when it detects motion or a loud noise.  I can also stream "real-time" video to my phone or computer and while streaming capture a picture or short video from the computer interface.

Is this able to be used outside?

You can use it outside or inside provided it gets power supply ( as it doesn't have built in batteries) and it's within WiFi range or Ethernet wire attached. Ethernet connection better than wifi
