I don't know about Asrock Pro 4. I installed it on an Asrock Q1900M, it fits but it is not working.

Does this work with Asrock X570 Phantom Gaming 4?

Yes but you dont need it on your motherboard. Just use CPU fTPM. Scalpers are trying to sell a $5-$10 item for crazy prices.

Does this include the connector?

The TPM has a female end that connects to the motherboard which has the male pins, but then you need to look at your manual to see if you have a TPM socket.
