How far from the wall does this extend?

It depends on the width of the TV.  We have a 24" flat screen and if you fold it in on itself it stands off the wall about 10"  It swivels and bends and tllts, very accomodating if you want to watch, swing over for a better view of an excercise video or if you want to do something in the other side of the room like play video games while someone is reading.

Question 2

How does it mount to the wall? Presumably into a stud? How many bolts actually attach to the wall? Is it secure enough to support an LG 49" Ultra High Definition TV? Thank you!

I used three studs that came with the mount--one at the top,bottom, and middle and it securely held up my samsung un40e6000 which only weighs around 25lbs. With the three studs, the mount feels very secure and i'd be confident up to around 40-45 lbs.
