Is this drive noisy?  In other words, does it make loud clicking noises or any other noises for that matter?

I have two 4TBs running in a NAS and I haven't noticed any loud sounds. They seem very quiet to me, however I'm not always writing and reading at all hours of the day too.

Can I put this in an external HD enclosure and run it with USB 3?

Yes, no problem.  Just make sure the external enclosure supports 4tb.  The Rosewill case with fan highly recommended.

Is this a bare drive or a retail package?

It's just the bare drive with no SATA cable, etc. You have to stick it in a drive bay and wire it up. If you are replacing an existing drive, you re-use the existing connections. Checked website url shown on bottom of package and found that Toshiba does not offer a manual for this drive. However it does have a tutorial for replacing your old drive with the Toshiba drive.

Is it 6-32 or m3 screws?

6-32, it has the same standard size screw holes as my other 3.5 inch desktop HDDs.

Will this work for raid 5?

Sure, as long as you get 2 more.  Need at least 3 drives for a RAID 5, other than that, brand doesn't matter, just match size.
