Does the packaging contain the rack mounts for this unit? If not, how do I obtain them?

It does come with rack ears. It's just two metal brackets. You can see the mounting holes on the switch in the photo.

Does the product implement QoS?

No, this is an unmanaged switch. Don't bother with it, mine ran just until the warranty expired then decided to stop working.

Does anyone know the POE output voltage?

802.3af is specified as 15.4W at 37-57 VDC, 48 VDC nominal 802.3at is specified as 25.5W at 42-57 VDC, 48 VDC nominal These voltages are required by the standards. Any 803.af/at device drawing power will support these voltage ranges.
